Registration Package
Are you interested in registering for the 2010/2011 school year?
You will need the following forms:

Registration Form (one per child)

Registration Forms will be accepted starting the first week of June. Contact your YMCA SACC Office for exact dates.
Does your child have an Anaphylaxis Allergy? Please complete the following TWO forms and attach them with your registration package.

Does your child require medications that will need to be administered during program time (ex. inhalers, allergy medication, etc.)? If yes, please complete the following form and attach them with your registration package.

Notes regarding changes:
SACC requires two weeks notice from the day the Change of Status form is received in the SACC office.
